Nail Fungus Treatment Options: Prescription, OTC & Cosmetic Categories

The right option depends on diagnosis, severity, health history, how many nails are involved and whether you are treating infection or simply trying to improve nail appearance.

Prescription topical options

Dermatologists may use efinaconazole, tavaborole or ciclopirox for selected cases. These typically require daily or regular application over many months.

Oral options

Oral terbinafine is commonly used for toenail onychomycosis when appropriate. Oral medicines can interact with other drugs and may require lab assessment or monitoring, so they belong in clinician-directed care.

Procedures and nail thinning

Debridement, trimming or other procedures can reduce thickness and sometimes improve topical penetration. These are not the same as cosmetic filing at home.

Non-prescription products

Some products are marketed for skin fungus, some for nail appearance, and some blend oils with antifungal-sounding claims. FDA's OTC topical antifungal monograph specifically states that the covered skin antifungal products are not effective on nails. Read labels carefully and distinguish skin indications from nail indications.

Do not stop prescribed therapy early because the nail still looks abnormal. The nail plate grows slowly, so appearance can lag behind infection control.
